Mental Health Case Management
The focus of Mental Health Case Management is to assist people toward a goal of recovery.
Mental Health Case Management helps people to explore and acknowledge feelings and emotions that are often related to their experiences. Mental Health Case Management does not provide therapy or clinical counseling services but will provide short term, solution focused support to help you in maintaining your mental health and accessing additional resources. Greater awareness allows people to reflect on what is happening to them and consider alternative ways of managing their mental health to successfully avoid crisis.
Case Management also helps participants problem solve challenges and set goals in their life so people can live to their utmost potential within their community. Case Management provides regular meetings with participants to address rehabilitation goals and provide ongoing support and feedback.

To do this we use the SMART format SMART Goals are:
- Specific: Well-defined, clear, and unambiguous.
- Measurable: With specific criteria that measure your progress toward the accomplishment of the goal.
- Achievable: Attainable and not impossible to achieve.
- Realistic: Within reach, realistic, and relevant to your life.
- Timely: With a clearly defined timeline, including a starting date and a target date.
Regular meetings will be scheduled based upon need of the individual. A review of the Rehab Plan will be conducted to determine what work has been done and what the goals will be for the next 3-month period. Participant engagement and commitment to their recovery goals will be expected and valued when determining what supports could be available during that assessment period and the duration of support.
