Intake Requirements
Please read the following before applying for services at Sara Riel.
To access our services, please fill out an application form. Our application form can be submitted online, or printed off from our website and faxed, mailed, or delivered to our head office. Call to request an appointment with our intake worker if you need assistance with completing your application.
Sara Riel Inc. provides community-based supports to people who:
- Are 18 years of age or older
- Are able to attend services in Winnipeg
- Want support with mental health and/or addiction challenges that are diagnosed (by a clinical source) or documented (by a non-clinical source)
